Click Form ST-10, Commonwealth of Virginia, Sales and Use, fill out the form, and print. North Carolina Department of Revenue Letter (if applicable) Must clearly show: Notice Number, Account ID#, Taxpayer ID#, and Tax Type: Sales & Use Registration.
